Question about self assessment exams

couple of thinks: 1. qbank and selfassessement exams are 2 independent thinks 2. qbank and SA will expire depending on how much time you buy: minimum 1 month for qbank and selfassessemnt(SA) expire in 2 weeks(i am not talking about extending this period) more specific: you buy qbank for 2 months....you will be able to access your account for this period starting from the day you activate it. you buy 2 SA exams and you have maximum 2 weeks for each to take it from the time you activate them/it=you can activate one right now and take the SA exam before starting qbank and see from which point you are starting, second one after finishing qbank or even you can activate both of them AFTER your qbank account has expire....my opinion: activate now only your qbank....do all the questions at least once or even better twice; after that activate one SA, do a SA exam and see where you are....look at that time on your weak parts and rewiew those parts/read more from other sources; after you are done with qbank plus you improve your weak areas, activate the second SA and see if you have any improvements....good luck

Mechanism of Action of OCPs?

maybe this explanation will help you....let's go first with the normal hormone effects: day 1 of menstrual cycle=bleeding; at this point LH will stimulate estrogen production; estrogen is keep increasing in concentration until let's say a concentration X; all the way up until concentration X, estrogen has a negative feedback to LH; when estrogen reaches concentration X, this negative feedbaack 'switch' to a positive feedback and stimulate LH secretion which will give LH surge.....very high levels of LH will give ovulation(breaking of ovarian follicle) and formation of egg and corpus luteum.....from this point corpus luteum starts to secretes progesterone plus some estrogen; from here things get's different if egg will be fecundated or not by a sperm; now, the OCP are most of the time 28 pills....firts 14 pills contain estrogen only to mimic the physiological effect of a normal body but this concentration in blood of estrogen will NEVER reach that concentration X.....as a result LH surge does not happen and ovulation does not occure; the next 14 pills contain mainly progesterone to induce myometrial thickness and to produce menses(i believe the last 3-4 pills have no estrogen or progesterone just like corpus luteum which at the end of lutheal phase stops in production of progesterone because there is no beta hCG from syncitiothrophoblast); bottom line: OCP mimic normal estrogen and progesterone concentrations seen in a female monthly hormonal cycle EXCEPT the OCP estrogen concentration will NEVER reach that concentration X which could give LH surge=no ovulation.... hopefully it helps.....

you will not get your result today....i'm almost 100% sure; i post many times informations about this topic and i will let you know what i know: as a rule for everybody, your score day will come following the next algorithm: look at your exam date.....move forward on calendar and get the very next wednesday...from this wednesday ADD 3 more weeks.....on this third week you will get the result; now, you took the exam on dec. 23rd.....very next wednesday is dec. 29th...from this point add 3 more weeks which makes january 19th your score day....you will get your score next week on wednesday....have fun until then :)

hepatitis question

i have some questions regarding hepatitis....it might sound a little bit foolish but i'm confused; my confusion is about antibodies against HBV and HCV.... for HBV: they say patient is positive for HBs or HBc antibodies.....what does this mean? positive for IgG or IgM? for HCV: they say patient is positive for HCV antibodies....again, they talk about IgG or IgM antibodies? last thing: IgG antibodies against HBs means immunity; IgG against HCV means also immunity? IgM against HBs or HCV means infectiveness with hepatitis B and C respectively right? please help....thanks to anybody who wants to clear this concept for me.

hi there....this is how i think you should solve this problem: first of all, you should think that the gamets cell(sperm and oocytes) contain only 23 chromosomes(they are haploid; the vast majority of the cells in the body are diploid, aka contain 46 chromozomi but are 23 pairs, which mean you got one pair from 1 to 23 from father and the other pair from 1 to 23 from mother); when you produce gamets, logic you will 'pass' further to your kid only 23 chromosomes(aka one pair from 1 to 23); innitialy the cell which will finally produce gamets has, just like the other cells in the body, 46 chromosomes; normally this cell should split this 46 chromosomes exactly in 2 and give rise to 2 daughter cells with 23 chromosomes each; if, by any cause, this process of splitting is not working proparly(meiotic nondisjunction), the splitting process is wrong; the most common error is when 1 chromosome does not split(very rare when 2, 3 or more)....that means the daugter cells which should have each 23 chromosomes, will inherited a different number....the number depend very much if that chromosomes which did not split will 'go' or not in a daughet cell....if is present, that daughter cell will have 24 chromosomes and if not the daughter cell will have 22.....this is the most likely answer= option D...answer B is another option but is much rare because 21 and 25 chromosomes means 2 chromosomes did not split(again this is much rare comparing to answer D)

need help please...

i am an IMG and did not score very good on step 1.....CK it's on the way but i would like to get in 2011 ERAS program.....i need some letters of recomandation and wonder if it's better to get it from an attending or is OK from a doctor too? what do you think about volunteer-is it worth it or it's a waste of time? it will look good on ERAS aplication or has no value at all this volunteer time? i could get first in a volunteer program for 1-2 months and then going into an observership(by that time i should be able to have both CS and Ck scores and, by my calculation, i should be finish with those before july 1st).....thanks to anybody who can help me.....

a 47 y/o woman comes to the doctor because of difficulty sleeping for 2 months. during this period, she has been awakening at 3 AM and remain awake worrying about work, even though she is tired. she has stable angina and required stent placement 6 months ago and a second one 4 months ago. her chest pain started to recur 1 month ago and now she has it almost daily. evaluation shows no organic cause of her recurent pain. she works as an attorney for a large firm and reports that she has been giving more responsabilities during the last year. for the past year, she has not enjoyed her work as previously. she forces herself to go to work and has difficulty paying attention while she is there. she is a gourmet cook but no longer prepares food because she does not enjoy the taste of it. physical exam shows no abnormalities. on mental status exam, she has a sad and worried mood and a reactive affect. she is allert and orriented to person, time and place. she states that she frequently becomes despondent especially when she thinks about her heart disease. which is the most likely diagnosis: a. adjustment disorder b. dysthymic disorder c. generalized anxiety disorder d. major depressive disorder e. primary insomnia this is an offline question...i don't have the right answer; what i choose is different from the list of right answers and i don't agree with the right answer listed for this question.....if anybody has an ideea, please post it(with explanations will be even better)....thanks guys

well...got my score

hi guys...it's been a long and rough road for me reaching this moment but at the end it made me so happy; i own this forum a lot and since today i receive my score, it's time to give it back maybe will help other guys like me. here is my story: after struggling badly on step 1(got an 79/195), i decided to try a different approach: i read all the kaplan books+MTB(no FA) and then i bought 2 months of uw; i went twice to all the q's and after i did one more time the psychiatry(my weakest subject overall)+wrong q's; after my first pass through uw q's i took NMBE 4(mid december): got an 350/195;i was so scared and depressed so i postpone my exam.after nbme, i finish my 2nd rewiev from uw and then i choose to go with more q's; i did 2009 kaplan qbook(fantastic book), FA Q&A(1000 q's) and Lange usmle step 2(i did 200 q's). january 21st-exam; 2 days before i took UW SA-520/222 and 1 day before i did the free NBME/Usmle CD-got an 82% q's right overall; on february 9th i got my score: 99/230; for those of you who are in the same boat like me, my adivice might help you; in my opinion if you can do what i did, there is high chances you will get an score like mine: step 1:read all kaplan books at least 1 time(i read them once but i made a bunch of flash cards)+MTB for step 3(i don't know anything about MTB for step 2) step 2:do kaplan qbook(not qbank! do it right after you finish reading kaplan books and MTB) step 3: do the first aid Q&A;70% of the q's are subject wise and at the end there are 8 simulated tests; step 4: buy uw.make sure you go at least 1 time through all the q's(my opinion go 2-3 times) and read every single time the right and wrong answers. if you have enough money you can buy kaplan qbank too; my budget was tight so i prefered to go to the medical library where i got some q's books for free. step 5: take an NMBE.if you are happy with the score, go to exam; if not do more q's(my plan was to do another 2000 q's from 'NMS step 2 ck' and Lange); unfortunatelly i had no more time in my opinion: if you go 2-3 times through kaplan, read all the UW q's 2 times +- kaplan qbank, do kaplan qbook and FA Q&A(read all the explanations from books), you have almost 5000 first time q's; if in general you score above 75% from the books and above 60-65% in UW/Kaplan qbank, there are very good chances you can achieve an 99. my preparation time: about 5 months(first 2 very poorly and only during the night-maybe 3-4 hours/day because during the day i had to take care of my son; starting with end of october, my mother in law came to US; she took care of my son so i study during the day; from this moment, i study minimum 10 h/day(average 12-13 h/day); i only took off 2 days for thanksgiving, 2 days for christmas and 2 days for new years eve when the library was closed). my scores: UW qbank: 55% first time and a 46 percentile; second time i was arround 67% and 75 percentile FA Q&A: subjects: 76.06%; SA: 75%; overall: 75.67% Kaplan qbook: subjects: 72%(no SA in the book) Lange Q&A usmle step 2 ck: 75%(i did 200 q's from the SA); NBME 4: 350 with an extimate of 195 on real exam(this was done after i went once through the UW q's); UW SA: 520/222 done 2 days before exam(january 19th) Free NBME/Usmle CD: 81.75%(37,37,35,3) one day before exam exam date: january 21st real deal: 99/230 received on february 9th(i am soooooo happy ) :) at the end i really wanna say thank you to everybody who helped me in this forum; thanks to my wife, to my mother in law who helped me a lot and the last but not least thanks to my son who was always in my mind(he really inspired me). Thank you GOD for giving me strenght and power to achieve this;with HIM next to me everything is possible!

well...got my score

my opinion: 40 days of not studying anything is quite a long period but not long enough to forget everything you already learn; if i would be you i'll go like this: read one more time the entire kaplan books(i would say no more than 1 months)+MTB(i did not use FA so i can't help with that)....then do the kaplan qbook(you get only 850 questions but are very very nice and well prepared-just like UW); next i would do FA Q&A....you get another 1000 questions; if you do about 150 questions per day reading all the explanations, both kaplan qbook and Fa Q&A will take you about 14 days(studying for at least 10 hours daily).....after you finish this you can go with UW qbank(i'm not recomanding this for you because you already bought it and unintentionally you will get a bunch of questions from your previously time) or go with kaplan qbank(best choice for you); make sure for either qbank you use to go at least 2 times through all the questions(i would suggest to go 3 times through your weak areas)....if you do this in a 6-8 weeks period, you will be able to take the exam in about 2-2 and a half months from now; the only thing is why i am suggesting to do so many questions is because as many as you do, it will be much easy for you to catch the most important things from a vignete in the real exam(you could read tons and tons of books....if you don't know how to approach a question, you will have problem with the time, understanding what they are asking you, etc) if you follow what i'm suggesting, you get 1 entire read of the materials, you get 850 questions from kaplan qbook, 1000 questions from FA Q&A and another 2200 from UW/Kaplan qbank....totally will make up to closely 5000 first time seeing questions; if you score constantly above 75% from the books and above 65% from qbanks, you can get a 99 pretty good.....hopefully it helps.

[quote=kasem]hi i took ur advice im reading kaplan books and mtb and doing kaplan q bank for each subject for example peds kaplan peds mtb peds q bank in a month i will finish it all then i will do usmle wolrd q bank for 20 days mixed mode for 2nd time do u think this way is good please advice and thanks because im really scared i heard that the exam is more harder but u give me a lot of hope with ur experience ...[/quote] hi kasem....regarding your studying method: everything is good as long as you are learning and understanding the books; i saw many guys doing this and they had success; but in my opinion you should do many many questions; i've been stressing all of you and i will still do it but the ideea is to do as many questions as you can; bottom line: now you are reading kaplan books+MTB and after every chapter/subject you read you do kaplan qbank(i hope i got it right); this is excellent; after this you said you will do UW; i would highly sugest after you are done with the readings+kaplan qbank to do the FA Q&A and kaplan qbook; in my opinion, do right now what are you doing(books+kaplan qbank) then go with ONLY FA Q&A and Kaplan qbook and at the end finish strong with UW; you are doing questions from kaplan Qbank....this will start giving you confidence about how to aprroach them.....after that keep building on that confidence by doing more questions from FA Q&A+kaplan qbook; at this moment you should be able to do at least 3-4 tests per day(i'm talking about 150 and plus questions per 11-13 of hours of studying daily-from those books you get about 1850 questions and with 150questions per day, this will be done in less than 2 weeks); you will see that at this time some of the questions will seem ridiculously easy; after this do UW; it will make you feel so good and be confident that there are very few things that you don't know by this time. why i am keep suggesting FA Q&A and kaplan qbook? this books are not that hard as UW but they focus mainly on high and medium yield topics; from my point of wiev, they filled many many gaps in my knowledges; UW is fantastic but in my opinion has too many hard questions which are rare in the real exam(from my experience i can say that the exam is not focusing on hard questions; you will get a few of them but most of them are medium and high yield topics; for this, kaplan qbook and FA Q&A are 2 excellent sources); i wish you good luck and keep trusting yourself.....i know a lot of people say that this exam is hard but if you study hard, you will be so happy at the end!

I already have MTB2.....i don't remember to say that MTB 3 is better than MTB2; what i said was i USED MTB3 and i can't tell anything about MTB2 about KAPLAN Q BOOK 2009 .i have 2006 edit......from what i know, 2005/2006 edition is pretty much the same with the 2008/2009 edition(2008 edition is a reeditation of the old ones with maybe some few extra things added) can i do nbme4 now.....i suggest you not to do it right now; NBME 4 is the lastest and from what i've read the hardest one....so if i would be you i would take it after i go at least 1 time through UW(if you really wanna take an NBME now to see where you are, take another form....1,2 or 3) FLASH CARDS will help me if i dont buy MTB3....flash cards are very good with or without any kind of additional source of studying; i don't know anything about buying them but i wrote them by myself. WOULD YOU TELL ABOUT HY TOPICS OF PEDIATRICS &SURGERY.iam just reading KAPLAN NOTES 2008-2009.....in my opinion kaplan surgery and pediatrics are enough for step 2 CK; maybe other guys will recommend you something else but i did not use something else besides kaplan and MTB i will do USMLEW QBANK/KAPLAN Q BANK before the real exam could i manage my study in a proper way with all subjects and all blocks in bothQ BANK.....what i did with UW qbank was untimed+subjects wise at first view and at second view i did them timed+mixed; some say is good to do tutor mode; this is up to you and how you feel more confortable about doing them; but in any way you are doing them, make sure you read ALL the explanations given(good or bad answers) where did you read ETHICS/PREVENTIVE MEDICINE.....kaplan and MTB; maybe somebody can help you with this and will suggest you an extra source because this is a highly tested topic(especially preventive medicine); besides kaplan and MTB i can't suggest you anything else because i did not use something else. about me....i post my score for step 1 and ck; so, next is CS(in march); in the rest i'm doing fine and studying for CS; all the best and good luck!
