The book says choice C is the answer. Explanations as stated in the book are
Choice C : The wife has clear evidence of abuse and has a well formulated plan for killing her husband; the spousal abuse and the plan to kill husband must be immediately reported to the proper authorities.

Choice A and B: Calling the husband only or calling the authorities only on the husband's behalf would be an acceptable course of action if the husband were not abusing the patient. Often abusers escalate if they feel that they are some how threatened. In this case, the wife needs protection from the husband, and simply calling the husband may cause him to become more violent.

Choice D: Reporting the abuse to the authorities is appropriate and mandatory. In this situation, though, reporting the threat of violence against the husband is also necessary.

( the book also mentions that spousal abuse in mandatory to report only in some states)

I agree with choice A. Child abuse and elderly abuse is mandatory to report but not spousal abuse. In this case what a doctor has to do is to prevent the husband from danger.
