Guys, need help

I am an IMG from India , studying 3rd year of Med school.
I am trying to apply for step 1 now. While filling the online application form for step 1, there is something called the "dates of attendance" in item number 18. I am required to do one year of internship (compulsory) and one year of social service (as per the rules right now but these May be amended by the time I finish my 4th year of med school).
I joined the med school on August ,2011.
I completed two years of med school and entered the third year.
I am going to complete the 4th year on March , 2016. After this I have to complete my internship and social service.
It would be 2018, March by the time I complete social service.

Now , what do you want me to fill in the form in the following sections:
