can any body tell me how to write findings of mini mental scale on patient notes

Hi,

Psychiatric mental status exam:

Patient appears well groomed
Alert, oriented to place, time and person.
Speech is fluid and goal directed.
Recent and remote memory are intact.
Attention and concentration are unimpaired as tested by spelling “cat” (or other) backwards.
Mood is euthymic. Affect is consistent with mood.
The patient does not have abnormal perceptions such as hallucinations, delusions, or paranoias.
He denies having suicidal/homicidal ideation or intent.
Judgment/insight are intact.

Or to make it shorter and when it's not a psychiatric case:
Alert and oriented x3, spells backward and can recall 3 objects.
