Who does the initial vital signs?

initial vital RN have to do it. follow vital for stable client uap can do it. read the question again bc you might miss the sterm

It was a SATA question about blood transfusion and 1 of the choices was to do the v/s before the blood transfusion I did not pick that and I was wrong... have you come across that question?

I do not remember but you have to take vital before and after blood transfusion you need a baseline.

i came across this question and yes the UAP can take the VS before and after the blood transfusion. RN will be taking VS during the first 15 mins of transfusion every 5 mins. hope that makes sense.

This is for blood transfusion RN need to check every 15 mins for blood reaction. Initial v/s is not the same thing. remember RN always do first assessment before delegation. UAP can only do v/s for stable client or critical client after RN initial v/s.
