Pathology question, help please!

The question was about ATN after septic shock with some gram - bug.
Weather regeneration of tubular cells will be possible or not??

In DR. Goljan's book it states that 1. after hypovolemic shock (hemorrhage)basement membrane is damaged , and not possible regeneration.
2. Nephrotoxic tipe that he mentioned is from Aminoglycosides, Radio contrast, and heavy metals, basement membrane is not damaged, and tubular cells will regenerate properly.
DR. Goljan didn't talk about bacterial septic shock.
Will regeneration be possible, or not.
I would like somebody to help me with that, please.
Thanks, Dusha

Hi,
I never read (or may be i m not remembering) that ATN is irreversible. till i know, most ATN are reversible.
Thanks for putting this new info. in my stupid brain....

IF hypovolemic Shock mediated ATN is irreversible than i think , ATN due to Gm-ve bugs will also be irreversible.

Reason : Gm-ve bugs -> Endotoxins --> Septic Shock --> Severe Vasodilation --> Less perfussion of Kidney (as like hypovolemic shock).

While Nephrotoxic is due to Direct injury to Tubular cells & so might be reversible.

That what i think... If anybody else have some other opinion pls let me know
